Pubialgia

"Pubalgia, pubialgia or pubeíte" are synonymous terms to identify the pain in the area of the low abdomen, where the musculature interferes in the "pubis" bone, in the pelvis.
The origin of the problem is the excessive requirement of the abdominal musculature, necessary to stabilize the torso when kick.
This fact, added to a retraction of the thigh's posterior musculature, commonly found in soccer players, originates an inflammatory process of difficult solution and take it to a chronicity.
Many times we also have an unbalance between the abdominal musculature and the aductur muscles, what has to be researched to apply the treatment.
Since the necessary rest for the treatment cannot be accomplished once an athlete's to keep on training, the inflammatory process is not solved, and an area of fibrous tissue is created that, after some period, won't react to ordinary treatments.
Some other problems can disarray the diagnostic so they must be dismissed. Abdominal hernias can cause pains in the area, and have a direct relation with physical effort, being the first pathology that should be dismissed.
We should also remember the infections of urinary treatments (cystitis, uretrites, prostatites, etc), that eventually can cause pains in the area of the low abdomen confounding the diagnostic.
In male athletes we must remember the problems in the testicle area (varicocele), and in female athletes the ovarium and uterus pathologies must also be dismissed.
For sustaining the diagnostic, we can use the magnetic resonance of the area, that will show an area with a located inflammatory process, close to the pubis (bone of the abdomem).
The treatment is traditional: trainings removal and physiotherapy sessions in the area, objectifying to relief the pains and extend the thigh's posterior musculature and leg; then, later, extend of the abdominal musculature.
In chronic cases, if the traditional treatment doesn't take effect, a surgery can be necessary to remove the formed cicatricial tissue and liberation of the abdominal musculature that comes reserved, with good results and return to the activity in about 2 months after the procedure, with total re-establishment of the athletic conditions.

Lightweight Sebastian Pereira, World Junior Champion and fifth placed in the Atlanta Olympic Games, had the first symptoms in February '98, during the trainings for the European Winter Tour; as he couldn't stand back for an appropriate treatment, a surgical intervention was necessary, accomplished in the last Aug 29th.  The pubialgia should be treated in the initial phase, with the athlete's removal for, at least, 3 weeks and with a carefull return to the tatamis. Otherwise, there will be a great tendency the chronicity.
